I don't know, it seems just a bit too "perfect" to me.

The text I got says they named the dog "Peter."

There's no reason a marking like this couldn't be real. I know a Ridgeback with a perfect "S" on his chest, as well as a beagle with an arrow on her rump pointing directly to her tail. Plus, there are those cows with markings that resemble Mickey Mouse ears and the outline of the United States.

I am by no means a photoshop expert, but I'm seeing a lot of what I guess I could call "weirdness" around the outside of the "penis" shape. Like they made some of it white that wasn't before. So maybe they didn't draw the penis on, but shaped it out of what was there.

Or it could be real.

Ha ha! I saw that picture on Facebook and hadn't even realised it was a dog - I thought somebody had made a plush toy penis for... reasons of their own.

While in Portugal , we came across a shop that sold religious images, icons, statuettes etc , and in the window was a giant wooden penis..

I was going to direct you to the site for Dildoll, but it seems as if the production of those was halted sometime in early 2008, and the WayBack Machine seems flaky right now. Though, it seems as if this site (NSFW, of course) has a couple left for sale. Etsy seems to have a number of plush phallic goods for sale, too, including cat toys (NSFW, too).

So, yeah... I guess the point of my post was to show you that this kind of thing is more common than you probably thought.

I can't say for sure, but my guess is fake. The shape of the markings just doesn't look right to me, it's too even. They look like maybe someone took a roundish spot on the fur and cloned it to make overlapping spots.
I also think it is somewhat unusual for that type of dog to have such symmetrical markings on both hips like that. It's not impossible, and JRTs and similar terriers do have markings that make a "cap" on the rear end like that fairly often (example, my Rat Terrier mix) but it just seems too perfect. I'm wondering if maybe someone just edited the markings slightly.
