First question. Why was there someone working in a elevator shaft with operating elevator? Where were the safety procedures and why were they not followed? I would think Captain and crew need to do a lot of answering of questions for that last one.

Any company certainly has safety procedures, but not all have a safety culture. And sadly, some people cannot be bothered. Just see how many people don't even wear safety belts in cars.

I'm sure someone is investigating this incident and I hope people learn from it and change their behavior.

From what I've gathered about cruise ship corporate mandates and on-board operational norms, nothing will be "learned" except to better cover up the usual safety failings.
