Airplane Report: Chain of problems behind Air Canada near-catastrophe at SFO

I thought we had a thread about the original incident, in which the pilots on an Air Canada flight mistook a taxiway for a runway at SFO, but I can't find it so perhaps we didn't. Anyway, the NTSB has released their report on the incident. It's noteworthy that this is the first time the NTSB has conducted a full investigation of an incident that didn't result in any injuries or damage. According to the CBC article the plane came within 3 to 6 meters of colliding with one of the planes on the taxiway after they initiated their go-around.

A chain of errors nearly ended in catastrophe for hundreds of passengers packed into five planes at San Francisco International Airport in July 2017, when the crew of an arriving Air Canada flight mistook a taxiway for a runway, federal aviation officials said Tuesday.
But at the last second, the pilot of Flight 759 realized the mistake and pulled up, coming within just feet of one jet to narrowly avoid what could have been one of the worst aviation disasters in history.

I think this will become a very important incident for many people to learn from.

I can see a lot of things that can be improved to prevent this from happening. Good thing the NTSB sees even more.

Most catastrophes have a chain of failures. DW has a book for her work titled "What Went Wrong" about failures in the chemical industries. Some of the chains are very interesting.
