US: 4.8 per 100k

A few others:

Germany: 0.8

China: 1.0
UK: 1.2
Lebanon: 2.2 (who'd a thunk it)
US Virgin Islands: 39.2
Afghanistan: 2.4
Australia: 1.0
Italy: 0.9
Japan: 0.4

I can't find the sky high numbers except in the harshest developing nations and Mexico.
