Fire in the Hole at Silver Dollar City in Branson was killed by a
head-related injury in 1980--as kids we had always imagined that it was a decapitation (and from the description of the accident, sounds like it very well could have been; plus it added some spook-factor to the ride) but the sources don't confirm it.
